Re-Enrollment Process for Students from Families Currently Enrolled at Avicenna

1) Call the main office to make a re-enrollment appointment.

2) During the re-enrollment appointment, the following will occur:

-a tuition contract will be presented to your family, based on income

-financial aid may be requested at this time

-you will receive information regarding vaccinations, supply lists (if available), an  

-academic calendar and a copy of our code of conduct that must be signed and turned in prior to the first day of school

3) If you are petitioning for financial aid consideration, you must submit last year's tax return as well as a document stating your income, monthly expenses and requested tuition amount.  Please be aware that financial aid is need-based and will not be considered for families that do not exhibit need. If you are applying for a voucher or tax credit scholarship, other documentation will be required.
